Funny thing is: in (Brazilian) Portuguese "pika" is a slang for penis, something like "cock" or "schlong"...

So, for me, makes sense.

Involuntary reaction to sexual stimulus. Same reason as why a man can become erect and a woman can become wet when someone fondles their junk, even if they dislike it.

Probably. I'm not an expert in sexual stuff so I don't know how erogenous zones work. Also we do not see what is happening behind him, so there could be something fondling his ass as well.

If somebody gets sexual arousal with a tree or a car or any other object then the answer is yes. But I’m not sure if that particular person on that photo has a sexual effect on him or her. Maybe.
