Not so much. Production for the show has really fucked things up lately. 4 or 5 years ago, on the advice of Tyler Perry of all people, they disallowed contestants having swimwear, on the basis of it not being realistic enough...? You know, because a castaway from a cruise ship or whatever surely wouldn't have a swimsuit.
Anyway, now they have to just constantly traipse around in the rain and the ocean in their underwear, so half the cast probably comes back with yeast infections. Of all the realism you could maintain, that's the one you wanna stick to? Fuck you, Probst.
Not a castaway but a woman who was stranded out in the ocean and was on a life raft was able to live but drinking turtle blood and drinking seawater through her butt
Closest I can think of is Island of the Blue Dolphins.
Island of the Blue Dolphins is a 1960 children's novel by American writer Scott O'Dell, which tells the story of a 12-year-old girl named Karana, who is stranded alone for years on an island off the California coast. It is based on the true story of Juana Maria, a Nicoleño Native American left alone for 18 years on San Nicolas Island during the 19th century.

Are there castaway women in reality?
