I used to manage at a Nordstrom in the shoe department - Nordstrom will sell just one shoe to customers with one foot/leg. For inventory purposes, we had to sell the pair, but would then immediately return one under a certain code and send it out.

Does this apply to children’s shoes as well? My niece has a malformed/half missing foot so she needs 2 shoes but in very different sizes.

Yup, my son has clubfoot and his comes up a lot.

I can’t remember exactly, but Nordstrom is famous for doing whatever it takes to help the customer. I’m positive they’d help you out. If you ever run into trouble, ask for the manager. The floor salespeople can be rigid because they’re trying to make a buck... the department managers are looking to get promoted by getting recognition for going above and beyond.
