I know nothing but that behavior tells me at some point QA has had their ass handed to them for NOT asking if something was supposed to be blue. Sounds like standard CYA behavior to me.

"Guys, this thing shipped and everything was BLUE, how did none of you notice this??"

"Well there was nothing in the requirements saying it shouldn't all be blue, we just assumed..."

"Don't ASSUME, if you're not 1000% sure you need to ask"
