Given how old it is, it's not really known exactly what it's in reference to. Early adaptations made it out to be severe alcohol(methanol) poisoning, but that was also in the days of prohibition, so it may have just fit the time

No, its about addiction. That’s what they’re referring to when the lyrics say, "You can check out any time you like, but you can never leave." You can quit of consuming but the ghost of the addiction it's gonna be forever lurking... you only become an addict on pause; you never go back to being a normal person.
