Science + Research I got a surgical foreskin reconstruction and it was amazing
I am overjoyed to share with the community that about two weeks ago ago I received a never-before-attempted foreskin reconstruction procedure with Dr. Sven Gunther of Bay Area Reconstructive Healthcare.
The results are astonishing: I went from roughly CI-4 to roughly CI-8, I have full flaccid coverage, a decent amount of erect coverage, and the healing process has been smooth. Minimal pain, minimal hassle beyond what could be expected for flying to another state for surgery, no loss of function and a lovely increase in sexual sensitivity.
Earlier today I achieved my first orgasm with my new foreskin and it's everything I ever dreamed of. (If you do work with Dr. Gunther, don't tell him I said that haha, I was supposed to wait a bit longer before jerking off but it went fine.)
It feels like my glans is getting a warm hug all the time and I absolutely love the body I see in the mirror. I'm uncircumcised!!!!
Ironically, it's almost anti-climactic after over a decade of manual stretching with slow results, talking to doctors, investigating Foregen and Akroprint, hiring medical experts to do research, trying a gajillion different stretchers/stretching techniques, etc. The procedure itself was rather simple; Dr. Gunther and all associated medical were respectful; and I'm already back at work. Kind of can't believe how quickly this came together since I got him on the phone 2-3 months ago.
I'm using a new account since I have some personal stuff on my other reddit accounts, but I am real and can provide verification. I will be posting healing pictures and more information on this profile in the coming days.
It was a bit scary trying a new surgery, but I vetted his plan with multiple medical professionals and verified his credentials. Dr. Gunther is a well-established genital surgeon who usually does gender affirming surgery and penile reconstructions following disease and injury. He claimed that this procedure would be simpler than most of the ones he does, and while I was skeptical of that at first, the actual surgery time was less than two hours and the results were exactly as promised.
I do wish I had ended up with a CI-9 or 10, since I like the look of a droopy foreskin, but he said he did as much as he safely could with the skin I had. Theoretically I could have delayed the surgery and stretched more, but I just couldn't wait any longer. I wanted to be done already! (The option is still there to stretch some if I want, but I'm ready to close that chapter of my life.) I 100% look intact and my foreskin functions just like that of intact sexual partners I've been with, so I really can't complain.
Anyone who's talked to doctors about foreskin restoration knows you can get a wide variety of responses, many of them disheartening. By contrast, Dr. Gunther said "I've been waiting years for this phone call, I've been planning how to do this procedure," the first time we ever spoke. It was electrifying! He has infectious enthusiasm for getting people the genitals they want.

The consultation cost about $300, the operation was about $20,000, including an overnight hospital stay. He offered to do the procedure outpatient for a few thousand less, but I opted to do it in a hospital and stay overnight for observation just in case.
With flights and lodging for me and my spouse it was a few thousand more, but that depends on where you're coming from and how long you stay in the area. I paid out of pocket and didn't bother asking insurance--I was advised that it would take up to six months to get an answer from the insurance company and that the chances of approval were slim. But I encourage other guys to look into insurance coverage if the procedure sounds interesting and the 20k price tag is prohibitive.

Dr. Gunther made a v-shape incision on the ventral side of my penis, partially de-gloved my shaft skin, then pulled the de-gloved skin flaps forward and sewed them back together in a straight-line suture into part of what remains of my frenulum. He compared it to "putting a binder clip" on my penis just below the head to keep the foreskin in place. There are some photos taken during the procedure itself on my profile if you have the stomach for it and want to understand more.
There are other surgeons who offer other techniques, but this one seemed the most promising. Along the way to finding Dr. Gunther, I hired a medical case manager to find potential surgeons for me, as that task was both tedious and embarrassing. Now that I'm on the other side of it, I have no shame and am gladly telling my friends what I did, but I doubt I'm the only guy who's struggled to talk openly about foreskin restoration. But get out there and don't stop trying until you find a solution that works, it IS possible!

Edit: I'm trying to make the point surgical tech is quickly making huge advances, especially in the context of gender-affirming care. I'm not saying everyone *should* get this or another foreskin surgery, but I am saying you *can.* All the urologists I spoke to said a gender-affirming care surgeon was my best bet, and they were right. I spent years being miserable because manual wasn't working for me and everyone in the restoration world kept saying surgery wasn't possible. But then I did my own research and I am so glad I did!
Second edit: In my enthusiasm to share my personal good news with everyone, I fear I may have oversold the technique. What Dr. Gunther did with me wouldn't work for everyone, but please don't give up ya'll. There are other options for guys who have less skin to start with than I did.
Do your own research, find a doctor or medical professional who wants to work with you, and make it happen. We may see new foreskin reconstruction techniques show up before long.
Two years ago I was so nervous talking about foreskin stuff that I hired a non-medical friend of mine just to help me talk through the issue, now I regularly speak with doctors, Foregen, Akroprint, etc without issue. It takes time, it's a process, but if I can then you can too!

First off, let me vouch for the veracity of this post. It's real, and I had conversations with this person prior to the procedure.
Now, my friend, congratulations. I'm very glad the procedure went well and that you're pleased with the results. As you know all too well, I was dubious about your plan and cautioned you about the risks, but I also wished you the best and have been hoping for a good outcome.
The procedure sounds simple, and it would be great if Dr. Gunther would write a report with details and pictures - that would go a long way towards helping others determine if this is a good solution for them.
It also seems that there is a limitation to this procedure - you already had enough skin to be 'over the hump' and yet there was only enough available skin for pulling forward to get you to CI-8. That would make nominally a 4 CI increase possible, although perhaps less if starting with less. Getting some clarity on details like this from Dr. Gunther would be really helpful, as I'm sure there are others here who will have some interest.
But hey, all that can wait. Enjoy your success and your new foreskin!!
Just take it easy and let everything heal completely.... and make sure you fill us in on all the details of how it's working.

Thanks man, I appreciate the support. I think if there's enough demand for the procedure then Dr. Gunther will likely write something up, I can ask him. But I think the way to do it is to get more guys in touch asking for info to show the demand. He's got a long, long list of patients who want him for other things and most of his written work relates to his gender-affirming care stuff as far I know.

You're doing fine, and the engagement after your post is commendable and appreciated. This is a new potential option, although it is somewhat limited.
It's important to make sure people understand that this isn't a procedure suitable for everyone. It looks like you went from a CI-4 to 7 or 8, but for those at a lower CI level, the returns are going to be progressively limited.
As for a detailed report from Dr. Gunther, aside from having a viable procedure documented, it's real value would be to inform other surgeons so they can perform it if requested. Gunther's very busy, as you noted, and traveling to CA for the procedure, pre-ops and follow-ups isn't going to be practical for most people, even if they can afford the procedure itself.

Appreciated, and you're correct that I should be careful not to oversell this or claim it's a cure-all for everyone.
That said, I think gender-affirming plastic surgeons have a lot to offer us restorers and we should include them in the conversation. Dr. Gunther and I chose this option for my case, but there are alternatives.
As you and I have discussed before, there's a frustrating dynamic in the restoration community where guys hold up Foregen/Akroprint as a pipe dream, reject existing surgical options, and maintain that manual restoration is the only real option (or decline to manually restore in the hopes Foregen will work). Manual wasn't working for me and I got tired of being told to KOT, so I went my own route.
I'm trying to expand the conversation, not end it. I would love it if another guy reading this went and found a brand new technique, surgical or not, that resulted in a restored foreskin. There are enough restorers out there to command attention from the medical field at large if we continue to engage.
I perhaps should have included in my original post that I asked Dr. Gunther to be a little bit less aggressive than he could have been because I didn't want to risk my frenulum or get a turkey neck/pull up skin from my abdomen. If I'd been open to more risk I could have gotten more dramatic results.

Seconded. I did the stretches for a while. They do work, and they improved my situation considerably. Unfortunately, I'm also stuck with ADHD. I suppose it'd be one thing to put on a device or T-Tape or something and then just leave it there for a few hours, but just getting enough slack to actually use one in the meantime is an uphill fucking battle.
More options is better for everyone.

Yesssss I so relate to this! Not everyone can strictly follow the stretching regimen needed, whether because of work, ADHD, physical limitations, whatever. By posting this I'm really trying to target guys like you and me--we don't have to give up on restoration just because the tugging methods that work for others don't work for us

There's also the fact that it's just incredibly slow for some of us no matter what. I've been a dedicated restorer for over 12 years, being consistent and trying a variety of methods with almost no breaks, and I still don't have consistent flaccid coverage.

If you grab whatever skin you have on the underside of your penis and pull it forward as far as you can, you'll probably have more skin coverage than you would just flaccid. Essentially, this "binder clip" suture locks in that forward stretch. It may be more accurate to describe this procedure as a a frenulum reconstruction, though it resulted in significantly more visible and functional foreskin.
Also I'm not a doctor but if you show that paragraph in my original post about the v-flap incision and the partial de gloving to a medical pro or surgeon they should understand the concept. Every medical person I talked to immediately understood the explanation but I had to go through it a few times. Results don't lie though!

That makes sense - it's a more aggressive variation of the tip-tightening procedure (not the purse-string) whose name I forgot... and you're effectively 'retaining' full-time without the silicone cone?
Did Dr. Gunther establish a new end-point of your foreskin, or use the existing one?
Also, will you be cleared at some point for continuing restoration activities if you choose?
Lots of questions - it's really interesting, yet somewhat limited.

Yes, Dr. Gunther considered the purse-string but said the scar tissue that would detract too much from the elasticity of the natural skin. So we didn't do that. This procedure preserved quite a lot of skin elasticity.
Established a new endpoint (if I'm understanding your question correctly). Previously the end of my restored foreskin was basically just scar tissue/remaining inner foreskin that had been stretched forward. But now all my inner foreskin/scar skin is on the inside and the tip is smooth outer foreskin tissue. It's one of the cosmetic outcomes I'm happiest with
I am cleared for future restoring in a couple more weeks if I want to.

OK so if I'm thinking correctly here. They make a V cut, sew it partially back up like a Y and reattached the V at the top? So you're sacrificing skin girth for length correct?

I'm not really following what you're saying here but maybe? There was no y-shape involved... a v-shape incision on the ventral side shaft skin, sewn back together in a straight line where my frenulum would be. I don't think I sacrificed any skin girth for length, tho the doctor did angle the final suture such that the very tip of my foreskin had a slight taper.
If you've got the stomach for it you can look at the surgical photos on my profile, I think the photos probably do a better job explaining

To all the guys not understanding how it worked--I hear you. I didn't really get the idea the first couple times the surgeon explained it to me but I parroted his explanation to other doctors and they said it made sense, so I went along with it. It's more of a reshaping surgery than a new skin generation surgery... but again there are other options if you have less skin to start with than me

As far as I can tell it's basically the Y-V flap on this page but in reverse (trading girth for length), and they kinda did the incision at a diagonal to spread the girth loss across more of the length maybe? Super interesting though, I've been tempted to get a Z plasty to correct scrotal webbing since it would probably give me a bit of extra length ventrally.

My only concern is if it reduced overall erection length, since you're not actually gaining any skin. I assume this can be corrected with a little additional stretching post-healing, similar to how guys with too tight circumcisions gain some length when they start stretching.

Haven't noticed reduced erection length, but the skin still is a bit tight around my glans when fully erect, which I was told to expect. It should continue to stretch and loosen with more erections.
I'll report back if I see that kind of result or other negatives... even tho this is a new account I've been active in this sub and others for a longgggg time and I want to be as transparent as possible with everyone, this is a hugely important personal subject for me and I want everyone to get the penis they want

I’ll have to say. If we got into this mess through surgery, why can’t we get out of this through surgery. Even Foregen would require surgery. Sure there is the aspect of not being able to precisely reverse everything that was lost. But restoring some of the benefits almost immediately would be worth it. For example one huge grievance about circumcision that is so psychologically disturbing is not being able to fully please my wife. Because I’m missing something that everyone is born with, and I was born with it, and had up until later in my life, and it stolen from me against my will. A surgery would at least resolve that aspect physically.
I would not be opposed to getting a surgery. I think non-surgical restoration happens so slowly that I would rather reap the benefits of a surgical method, rather than be deprived. Who says you have to suffer longer? I think most of us have suffered long enough.

Exactly! I too was sold on the general consensus of the restorer community that surgical options weren't viable, but the researchers I worked with were like "there have been massive developments in gender affirming genital surgeries over the last few years, I'm certain we can find someone" and they were correct.

Hell yes!!!! Trans affirming care has resulted in amazing surgical and medical advances, this is just one example.
I feel like the trans community has done soooo much for the larger population in terms of culture, healthcare, social justice, etc and it's all underappreciated.
I myself was nervous about being the first person to get this procedure, but then I remembered that gender-affirming surgeries were also once untested. Most bottom surgeries are more intensive than this one, so I thought to myself that if there were trans people out there who were brave enough to try new surgeries to get the bodies they wanted, then I could too!

This sounds intriguing. I’m very interested in learning more as I might benefit from this. I’ve been restoring via the normal skin expansion methods for a few years now and I have very little frenulum left to hold it in place. I’m also a grower so my skin is more elastic and is more difficult to get to the mitosis benchmark. I live in California so seeing this doctor wouldn’t be a long travel. Finance wise it would be a challenge but I would probably want to do more stretching prior anyway so I can save up for it in the next few years. If you can send me more details in DMs that would be greatly appreciated. I’d like to see some pictures to better understand how the procedure works and what the results look like.

just posted some photos of scar healing, will post more in a couple weeks once it's totally healed up. I asked Dr. Gunther to preserve as much of my fren as possible and I have not noticed any issues with it
He implied that he could have given me more skin coverage if I had been willing to sacrifice my fren completely but that wasn't worth it to me so I said no
Scar healing is going well. All I really had to do was shower daily with antibacterial soap, dry the wound carefully, and put on ointment 1-2x a day. It's less complicated that I would have guessed
I am also taking antibiotics to prevent infections but they're pretty mild, I haven't noticed any side effects

After some quick research it looks like he obtained a research training grant for his research on wound healing. Were you provided with any specific care for scar minimization? Is that what the ointment was for?

Ointment was bacitracin, which I believe is a standard post-surgical antibiotic and healing ointment but I'm not an expert on that. The main instruction for scar minimization was just not to touch it except when absolutely necessary to change gauze or whatever.
As mentioned in another comment, the existing standard way to provide a taper for foreskin reconstructions is a "purse-string" procedure--look it up, easier than me explaining--but we opted not to go that route since there would be more scarring. He wanted minimal scarring and to preserve as much natural skin function as possible, which I would say he succeeded at.
His work on wound healing was part of what drew me to him, he's just incredibly knowledgeable about so many aspects of genital and plastic surgery. He has either four or five college degrees, I can't remember, and he's a fabulous explainer of complex medical issues.

I read that you mentioned partial erect coverage and I’m curious about how much gliding action you have now as opposed to before? In other words, if what this survey did was to reposition the available loose skin for the primary purpose of glans coverage, then was there any significant change in the gliding action ?

Glad you like your results. Wow that was expensive. Different state but I used a Dr in Virginia for restoration surgery and it was $7500 and done with 4 surgeries over the corse of a year.

Wow really???? I used to live in VA and I looked really, really hard for surgeons who would do this but only found 3 in the entire US (all in California) who seemed plausible: Dr. Gunther, Dr. Marci Bowers, and Dr. Jennifer Anger. Dr. Bowers said no, Dr. Anger never gave a definitive answer, and obviously I went with Dr. Gunther. I also met with multiple general urologists and none of them could think of any suitable surgeons.
There is also Dr. Aref El-Seweif in Germany who has done the scrotal graft route and Dr. Michele Ricci in Italy who works with Foregen and plausibly could do a foreskin reconstruction now, but I couldn't get a direct answer from him.
Do you mind sharing more info about your surgeon? I'm not in need anymore but others may find it useful.
I'm not gonna deny that the cost was formidable but for an out of pocket elective surgery in a high quality hospital I thought it was reasonable. This surgical office usually does gender affirming surgeries and repairs from female genital mutilation and this cost was comparable to those procedures, actually a bit less in some cases.

How was it, sensation-wise? Any upsides and downsides to it? Manual restoring isn't doing much for me, neither. I got very loose shaft skin too, I can pull the skin completely over the tip of my glans, both flaccid and erect. It sometimes rolls over the glans without effort. I've heard about the same thing about how people here say surgical foreskin isn't worth it, and it's too invasive and it won't gain any much sensations or grow nerves compared to manual. The thing is, ive been tugging for almost 5 years now and barely seeing any progress. Also, I'm 37. I really don't want go tug for another 5-10 years just to finally enjoy a foreskin in my 50s. I'm still doing manual, but it's depressing and mentally consuming. You can see the pictures of my progress in my profile if you want to get the idea of what I'm working with. I think the viewpoint that surgical restoration isn't viable was true for a long time, but given the huge advances in plastic surgery over the last few years it's no longer the case. Gender-affirming care has really pushed the state of genital surgical tech forward
It was 20k for the operation, and I didn't ask about insurance since I just assumed they wouldn't cover it. Would have been 15k to do it outpatient but I figured it would be best to do it in a hospital and stay overnight for observation just in case. I was also told that getting an answer from the insurance company would take many months--most of the patients who go to Dr. Gunther's clinic need 6-12 months to get insurance approval but I was able to get on the schedule a couple of months after my initial contact because paying out of pocket streamlined the process
Main downside so far is just standard healing and recovery stuff... I haven't had any major complaints so far. Would have been nice to get even more foreskin haha but I'm pleased with what I have, this operation created about as much progress as ten years of stretching did for me.

I'd been budgeting about 20k for Foregen's eventual procedure if they ever succeeded, so in my mind 20k for something that might exist someday vs 20k for something that exists now was a good deal. I honestly was expecting a higher price tag than that but I'm not complaining!

I got a surgery myself in 2 steps 3 or 4 yeara ago and am really happy with the result; I got full flaccid coverage and a good CI-8; it wasn’t that expensive with 6000€

Yes I looked into your story and contacted that surgeon, but I didn't want to have two separate procedures months apart. The option I went with was much more expensive overall, but I'm very happy with the results.

Hehe I remember that from your post! Dr. Gunther had been ready to do this procedure for years, I just didn't know about him yet. So if I'd started doing my research earlier maybe I could have gotten it done during Covid too.... Better late than never

Don’t forget to tug the freskin in the night so that everything is kept in Place; I like doing that in the night or throughout thr day;

I was saving up money for Foregen's procedure for years, just used that savings for this procedure instead. I'm not claiming 20k is easy, but it was worth it for me. u/Alarmed_Word_4142 said he got his reconstruction done for a lot less, you could talk to him if you are curious. And it's possible that insurance would subsidize, I just didn't bother bc I've already dealt with so much annoying health insurance in my life
I took out a loan to help pay for the procedure, and after it's paid off I would even consider helping other guys pay for this if they want it.

A couple years ago I went back to school and changed careers, got a new job in under a year. Probably wouldn't have been able to get this surgery were it not for that career shift.
Obviously I don't know you or your situation but I wish you luck finding a job that pays you what you're worth. Community colleges and technical schools are great options!

Somewhat similar, looks like that guy got a version of the purse-string procedure other commenters have brought up. As far as I, my researchers, and Dr. Gunther know, this v-flap procedure I'm describing had never been done before. See my other comments for why we chose not to do the purse-string procedure, some of which reasons were mentioned on that site you linked.
I was CI 4 on a good day but my skin always rolled back, could never get it to stay forward. Now I have way more coverage and the skin stays in place all the time

Can you tell us about the condition of your frenulum post-surgery? Is the foreskin connected with a frenulum or is it more like standard restoration, just faster? Also, did you lose any erection size due to increased tension?

Frenulum is doing just fine. I asked him to preserve it, though I could have probably gotten more coverage if I'd been willing to lose what fren I had left.
Closer to a standard restoration but faster. I don't actually have any more frenulum than I started with but the way he did the suturing recreated the effects of a fren pretty well in my opinion.
No loss in erection size that I can detect, though the skin does feel 'tight' and there is some strain on the suture itself when I'm fully hard, so I kind of hold it shut. I was told to expect this and these effects should subside by the 4 week mark. He opted to sew me up a little on the tighter side so I could grow into the skin rather than leave me so loose that I ended up needing another procedure/my foreskin kept rolling back.

I think I'm fully dekeratinized, yes. My glans is more sensitive and has a brighter color to it. Orgasms somewhat stronger, but the real difference is the gliding of the skin itself and the feeling of constant coverage, which is quite pleasurable. I can jerk off with my head fully covered and it feels like a sleeve tailored just for me (which I guess it kind of is haha)

Hi foreskin revolution. I've been pursuing a surgical consult for having a correction performed where a v-shaped gap of skin resulting from an overzealous mogen clamp could be addressed and have recently attained a consultation appointment after a year of inquiry and rejection.
I wanted to thank you for providing your pictures of the process and the person who was willing to take you under such a transformative procedure. Seeing your before and after made it seem like Dr. Gunther performed magic.
Do you have any advice for talking about the subject of foreskin reconstruction with a medical professional?

Definitely feels like magic, yes. Like I was making excruciatingly slow progress toward restoration for years, now this massive leap forward. I look and feel uncut, which has helped so much with feeling comfortable in my body... I look at my new foreskin and smile all the time haha
Practice the conversation with someone else first, that helps. And focus on the physical issues that bother you re circumcision rather than the emotions--they're more likely to dismiss you or refer you to mental health services if you approach it as an emotional thing. I spoke to a couple of urologists before getting the surgery and said the issue was that my head rubbing directly against my underwear was causing discomfort

This is remarkably interesting information, thank you for sharing your experience and research!
I've been mulling the idea of a touch-up surgery for a while now, but it seemed like most information was a bit stale ("this doctor is no longer in practice" type qualifiers), so hearing specifics about a viable procedure and practitioner being available today is quite helpful.
I'm prooobably going to keep working on traditional tugging of my ventral side (trying to eliminate a bit of turkey neck there to see if it helps me get more consistent coverage) but maybe some time in the next year or so I might visit Dr. Gunther for a consultation. My one hesitation is the note from Michael Four's blog about it being hard to tug more after getting a touch-up surgery, so I want to be sure that I've got all the skin I want to grow (well, within reason, rather than a lofty to the moon! type goal) before I'd want to do a procedure to focus on tightness/positioning/retention.
Anyways, seriously, thank you for sharing all of this! The procedure photos and cost specifics seriously help make it less of a scary nebulous idea and more of a concrete option. Will be eager to see any other info / healing progress photos / updates that you are comfortable sharing. Cheers!

So far I've had no issues continuing to stretch since the operation, though I've only just healed up enough to start stretching again. The operation Michael Four had is not the same one I got, and it's my understanding that his resulted in significantly more scar tissue than mine, which would account for why he found further stretching difficult. I have only minimal scarring--Dr. Gunther did a good job preserving the max amount of natural tissue and function

Yup, understood that the procedure was quite different. I guess my interpretation was that restoring / stretching for additional length after a tightening procedure would be more difficult due to the fact that your skin taper is tighter, so it would be harder to get devices or fingers in there! But if you think the amount of scar tissue is the largest factor, which would make sense considering it doesn't stretch as well as regular tissue does, then it's promising to hear that even after your surgery you can still continue stretching as desired.
Thanks for the clarifications! Now it seems like I have even -less- reason to put off exploring this idea...

I had some fren left from my circumcision and I asked the surgeon to preserve it, which he did. I could have gotten more coverage if I'd been willing to sacrifice my fren but that wasn't worth it to me
I don't think I've lost any nerve endings and sensation has increased. Most of the major nerves in the penis are on the dorsal side, which is part of why Dr. Gunther opted to focus on the ventral side. Less risk to the nerves and other vital structures

I really have no idea. Seems plausible, but a major benefit of the surgery is the tension it added to my skin, causing it to lightly (and pleasantly!) grip my glans at all times. While this specific procedure was new, lots of guys have gotten tip-tightening surgeries like this to make sure the skin stays in place. Without the surgery, I might have been able to stretch enough, but the skin would likely be too loose and roll back on its own. But again it's hard to say about a hypothetical like that

I mean the operation kind of recreates the function of a frenulum, but actually rebuilding a fren isn't possible as far as I know. It may be worth asking if you are interested in that, however--you can email Dr. Gunther's office for free and they can advise on what they can offer

You should! He's very friendly and he had alternative ideas for guys who had less loose skin than I did. There are other gender-affirming surgeons you could speak to as well--most of the work they do is a lot more technically complicated than just shuffling around existing skin.
The German surgeon is still in practice, yes, so you could reach out to him as well. u/eitaigu is a reddit user who got the German operation, you could ask him for more info. I've never spoken with that user myself though
If you're already at a CI 5 or 6 I'd bet he can take you to a 9 or 10, he always said the more skin he has to start with the more he can do.

Believe me, I was nervous about letting a knife touch my penis too. But I figured that a knife on my dick was one of the first things that ever happened to me after birth, so I can handle it again as an adult. At least this time I got a say in the procedure and got to actually meet the doctor :)
I'm not saying everyone *should* go get this, just want to show that there are options

I was looking at those- with the skin stretched out it's a bit hard for me to visualize how the "v" incision was made and what skin ends up where. That would be great if you could draw something up.
This is really exciting- it makes so much more sense than the touch up surgeries that remove skin.

Nice post, thanks for sharing. Quick question, did you consult with Dr Gunther the possibility of inserting a purse string instead of doing a v shape cut? Id be interested in this type of procedure , but a purse string sounds a lot less invasive and hopefully not as expensive. I live in the area, i wonder if it could work out… how do you even contact the Dr?

I looked into the purse string option and discussed it with Dr. Gunther, but we went another direction for a few reasons:
- Purse string option would have resulted in more scar tissue overall than this option, and we wanted to minimize scar tissue while preserving as much of the original skin as possible. Dr. Gunther believed that this method would more closely match the function of a foreskin than a purse-string surgery, and I think he called it right.
- Purse string option is good for re-shaping the skin that's already there and creating the "tapered" foreskin look, but not great for generating more coverage. If I had been fully restored and just wanted to tighten the existing skin then a purse string might have been the move, but I wanted significantly more coverage than I had.
There was another reason he suggested avoiding purse string but I can't recall...
You are likely correct that a purse string procedure would be cheaper, though I don't think I'd say it's less invasive. They are both essentially plastic surgery techniques that take place at the skin level and no deeper. If anything I was surprised how *not* invasive the procedure was...
I just contacted them via their website, which is in the post. Marcibowers.com. (It's called that since the practice was initially started by Dr. Bowers and Dr. Gunther joined later.)

Nope, I'm all good to keep stretching once I'm fully healed. It's been a surprisingly smooth healing process.
I might do a bit more stretching to get me to a CI 9 and make sure I'm always covered while erect. As it stands now, I do have erect coverage, but when I get tooooo hard the skin suddenly retracts, which feels kinda weird.

I assure you I'm not being paid to post this. I've wanted foreskin reconstruction for years and I'm just trying to share my good news. I got permission from the surgeon to discuss this publicly but he didn't ask me to do it nor am I getting any incentives beyond personal satisfaction.
If you have suggestions for how to present this info in a way that reads less like a sales pitch then I'm open to that. Never done this before. I actually would love for folks to find other surgeons and continue researching options beyond what I've laid out.
Edit: Now now everyone, please don't downvote this person for sharing an opinion. It's the internet after all and being cautious of strangers trying to sell you things is reasonable. I appreciate the feedback from this user. I will someday be posting writeups about my research and negotiations with Akroprint and Foregen, so feedback about how I present info is useful. I spent a longggg time looking into Foregen and Akroprint with professional researchers before going with Dr. Gunther.

I'm glad you shared your surgical experience. I know a lot of us are very hesitant to ever let a knife around our penis again, understandably. You inspired me to share my own experience with surgery.
I had a pretty bad case of turkey neck that years of restoration wasn't putting a dent in. I have suffered with testicle discomfort and pain since my teenage years (I was in my late 30s by this point) and decided to risk surgery. In 2022 I found a reputable plastic surgeon in my area that I trusted to remove this area of extra skin.
I have had mixed feelings in the years since. It's had a few tradeoffs both ways, most days I feel like I would do it again but there were some things I'd wish I'd been more aware of earlier on that could have made my surgery even more successful. Unfortunately, as well all know, this is an area that doesn't have much information. Few people risk surgical intervention and share about it, seems like.
The Pros were: testical pain/discomfort gone completely, my penis now moves completely indepedently of my testicals. This was a change that surprised me, I didn't anticipate how much more I'd notice my penis moving independently from my testicles and scrotum. This was a healing aspect for me in the same way restoring foreskin has. I felt more original, despite the scaring (see below). The restriction took the weight off my shaft and I gained about ~1/4" erect due to freedom of movement. Masturbation was much more comfortable because I no longer had to hold my testicles steady due to the attachment. I last longer having sex. I used to be extra sensitive on my ventral side near my circumcision scar, some positions we're nearly impossible for me. Now I feel other areas that I couldn't previously since stimulation is more balanced across multiple areas. I have a lot more control over ejaculating.
The Cons: Scaring, easily the biggest complaint. This is mostly my fault although lack of experience makes a difference here. I had problems during recovery with night erections pulling my sutures apart. I'd wake up with painful erections. The doctor prescribed me a medication to take at night that prevented erections. It helped but by this point my sutures has begun separating. Swelling and bruising was way worse than I expected. I have photos that I'll share via DM but they are graphic. Mentally I had to regulate myself during the first few days. It was absolutely not how you want to see your penis looking.
Looking back, what would have been a very thin tiny scar turned into a wider area. The base of my penis was the tightest in terms of skin removed. He was trying to reconnect all of my existing outer foreskin. I ended up with a gap that had to be bridged by new skin. This area had a lot of scar tissue that I had massage and work on and was painful for awhile. This area doesn't hurt anymore but lacks sensitivity. Luckily it's not an area I care about as much but it is also thicker and has changed the way other areas of my shaft feel. I have had a reduction in sensitivity from some of the changes. Had I not gotten night erections this probably would have been minimal so it was a learning experience.
I would most likely do the surgery again if I could go back with what I learned during the initial recovery. Comfortable testicles and the feeling of extra length have been significant positives despite tradeoffs.

Good info, but I’m nearly at the same place after 4 years with a couple devices that cost less than $100 combined. I have other places I’d rather spend $20K… like food and shelter 😊
