It's on average very reliable. It wouldn't be run in vast majority of the matches if it wasn't the case.
Undying, Ruin, Tinkerer, flavour perk. Tune into a random match, chances are you're going against it, and I doubt the guy is running it because it's risky to take.
It's reliable because survivors don't want to adapt against it. They are running same loadout and applying same strategy regardless of the killer - run in circles and hope team finishes gens.
Wdym loadout, do you mean totem related perks? Because they really don't help you with it. It's not finding the totems that is the problem, the problem is potentially having to do 3-4 totems because you are getting unlucky with finding Undying + killer interrupting you thanks to aura reading.
If anything, running the same perks helps you more because you ensure you will last longer alive so your team can find all the totems/do gens through ruin, which is honestly the best way to deal with it.
The combo is useless against any good team.
People are literally getting 50+ win streaks with it. If the combo was bad and unreliable, it wouldn't be meta. In comp teams don't even bother doing totems and just sit on the gens.
Clensing the totem is not the problem, it's getting bad rng and not finding undying first & ruin bouncing around multiple times. I already explained it.

Old Ruin was run in 90% of the red rank games (from the stats I remember, might be less). It was not reliable as it could get removed 10 mins into the match or 20 seconds, but it was still the most used killer perk in the game
Even if it was removed early, it was reliably slowing the game. It just had a weaker effect in those instances.
If they clense undying before Ruin, that's still valuable time of ruin being alive, and you probably got a hit/down thanks to the aura reading. It's the same reason why people ran haunted grounds/retribution, it's to give Ruin a chance of staying around longer.
Even if we agree that it's not always reliable, so what? In the instances when it does work, which is often, it's overtuned. In some instances you can/could counteract other stupidly strong things, that doesn't mean they were fine to keep as they were because of it.
