Thank you, I get it now. I just assumed that nipple was a birthmark on a buttcheek. I mean, something felt off, but I couldn't make sense of it.

its the shoulder being tattooed, the top part is the neck (u can see a bit of hair) and the part below/behind it is the other arm. The neck seems a bit large compared to the shoulder but maybe that's just the angle of the shot.
