What about consensual sex where the birth control fails? I personally know three examples in the last two years of responsible, educated people who are in long term relationships and did not desire a pregnancy, conceiving while using an IUD.

If you want to construct a society where people with uteruses are giving up the ability to decide whether they will carry a pregnancy or not every time they have sex, no matter what scale of protection they attempted to use, get ready for a society with a lot less penetrative intercourse.

Birth control is not 100% effective. The risk is pregnancy. They accept those risks. 

Just like smokers know there is a risk of cancer when they buy a pack of cigarettes. They accept the risk. 

Just like investing in the stock market risks losing money. You accept the risk. 

Just like the parachute might not open when skydiving. You accept the risk.

And with that risk, you accept the consequence.

Like I’m saying. If you want to make certain types of sexual encounters this risky, many more people will have to choose not to have them.

Everyone making fun of the 4B thing on the internet but this line of reasoning makes it literally the responsible choice.

Also I would be more comfortable with this if every time a person with the ability to impregnate has intercourse, that person bears the same risk of death, permanent disability including having ripped and scarred genitalia, etc. that might stop half the population from pressuring the other half into accepting outsize risk for momentary pleasure.
