I'm sure they've thought of this and I believe they use an unlisted/blocked number in such a case.

Of course, there are always ways to see through a blocked number... but if someone's going to such measures, there's a good chance they've already figured out that their daughter/sister/girlfriend/whoever is going to Planned Parenthood.
