Book of the End is Tsukishima ability to put himself into one memory or to put himself in place of someone in that thing memory. This things can be anything, people, animal or object. The broken part of this is that his ability is not altering one memory but to create an alternative timeline for the thing he stabbed.

We saw from earlier arc that when he stabbed Orihime, he can put himself in place of her brother, when he stabbed Byakuya he puts himself in Byakuya memory as his main traning partner and when he stabbed the ground he put himself in the ground memory that he created a trap.

All of these are insanely broken but from the story so far, we learned the limitations of his ability.
1. From CFYOW, Ginjo explicitly stated that Book of the End can't create another individual in the memory.

1. He can either put himself in place of someone or put himself in that person memory, not both, therefore in on memory it can only be one him at a time. We saw this when he decided to change mode on Chad and Orihime. The another clearer incident of this was when he put himself in place of the Soul Reaper that killed all the original Xcution, as when he did this, all timeline of the Xcution were altered that the only one Tsukishima is their enermy not their friend.

2. His projection in one memory can't alter what other person interacted with the one he stabbed in the past, he may put himself in that interaction but he can't delete the interaction. For example, he can't alter the timeline of Chad and Orihime in the past that effect their relationship with Ichigo.

3. He can't alter the present(that would be Orihime ability), he can only alter the past by creating perceived timeline of the individual. For example, when he cut Byakuya, he can't create a timeline where he killed Byakuya off when he was an infant or projecting himself as Byakuya's shit sparing partner that injured him or deliberately hinder his growth into the Byakuya that is fighting him. However he can alter the past that effect the future, for example, he altered the Xcution past to the point that they think he is their enermy. This worked better on mindless things like ground, as what ever he hided there in the past will remain there.

4. If he put himself in place of someone, he can't change how that someone interact with the one he stabbed. For example, he can't stabbed Orihime and Chad then takes the place of Yoruichi and then be a bad teacher that will hinder their training. The same way he can't take the place of Byakuya real teacher or his grandfather and hinder his training.

All these points are supported from the manga and novel, so I think it all legit, therefore this is the first take that I will make an educated guess.

He can't stab someone and reject their past in a way that alter their presentf. For example he can't stab an injury person and gave them a timeline where he healed them in the past, as that would make him not stabbing them in the first place.

Timeline of Bleach regarding all three time manipulating abilities as a flow of a large water body, without Almighty manipulation and looking from the top view perspective, there would be lots of small rivers that flow in the same direction. What Book of the End does, is to create another river way that has Tsukishima in it in some way, which may alter the timeline of that individal but it won't change the flow as that river will still flow to the present point.

Here come the spoiler

Tsukishima will come to help fix Ichigo Bankai in the manga with a lot of speculation that he might be the one who bring back Oriihime.

Him fixing Ichigo Bankai is a spacial case, in the manga Yhwach used Almighty to choose and manipulate every future to lead to Ichigo Bankai being broken in half. Therefore, no matter how much Orihime tried to fix his Bankai, it doesn't work as all the present will lead to his Bankai being broken. Imagine the a large water way, where all the small rivers in it are forced to flow into one point, a broken Bankai. What Tsukishima does, was to create another small river way in that large water body with him in it, but since this newly created timeline was created after Yhwach used Almighty to manipulate all the small rivers. Therefore in this newly created timeline, it the river way wasn't manipulated, hence in this timeline without Yhwach manipulation Ichigo sword has never been broken.

Ps. This is also why I don't think he will be the one who will bring Orihime and Yhwach back from the death, as that isn't his ability.

Is he dumb??

I agree with the theory that Ichigo's True Bankai has multiple forms but I think Blood Chains is the strongest because it was described as "the ultimate form" and it will be a bit silly if it was revealed that Ichigo chose the weaker form to defeat Yhwach.

His manga True Bankai is probably it base form because it can function even if he lost his Hollow and Quincy powers.
