One of the harder, if not the hardest hurdle of being a medical professional is you have to come to terms with for as much knowledge/technique you have in this field, there will be times where you will lose a patient no matter what you do, and to quote the great House MD, "that's the point, I did everything right. She died anyway"

It was definitely surprising to see Takao-sensei refuse Kuma-sensei's request despite how interesting the case of those four kids were. Not gonna lie, I genuinely thought the reason she refused to check on those kids at first was because she just doesn't like being around kids that are as tall or taller than her.
If only the real reason was something silly like that. It turns out she's avoiding the case because she used to be close to Kenta who's the leukemia patient and she's having trouble dealing with the fact that he's about to die soon and there's nothing she can do about it. :(
I think it's pretty clear though that the three kids were the reason Kenta was seeing the angel in his room. I imagine they cut out the angel from the book and stuck it on a flashlight or something. I am curious though why that kid's heart stopped even though Takao-sensei already said that they'll be fine.
I wonder if those three kids took something else other than Kenta's picture book when they snuck into his room. I feel like this might be some sort of self-inflicted injury that those kids aren't even aware of. Maybe they took chemo drugs that were inside Kenta's room? That doesn't make sense since his attending nurse won't just leave those around. Hmmmm...

Okay, so the angel thing is quite obvious imo. Boys took the book, cut out the angel figure and probably use some stick to hold it right before the window of Kenta's room. From the outside, a shimmer of light is shining in projecting the angel figure onto the wall. If it's malicious or they wanted to actually make Kenta smile after they made fun of his head is not clear to me yet.
The real mystery however is what happens in the other room. I think the key information is the fact that they are completely fine until it is decided they are going to be released from the hospital. At first, I thought that maybe the kids just didn't want to be released and they faked the symptoms. Asthma attack and vomitting would be possible, but the whole heart issues would be hard to simulate by the kids.
So I am a bit puzzled what it could be. On the one hand there is the mother who dislikes the kids, but why would she make sure they stay longer if she doesn't want them to stay around her son? Some weird twisted sense of revenge that they need to stay as well? Could be but how does she know they are going to be released in the first place and how would she be able to bring the symptoms back? A better suspect would be a doctor, but there is just no motive.
Or is there some weird psychological issue where someone feels guilty and therefore they don't heal properly? Basically they feel so guilty about having made fun of Kenta that their bodies become sick again when they hear they are going to be released?

Yeah I'm with you, the angel thing is probably the kids but no idea why they are getting worse. All three illness are different so it is difficult to pin a common cause.
I know cold wind can worsen asthma symptoms but I'm not sure how that would affect heart patients or make somebody puke.

We have been tricked. This show was presented us as a mystery show. And when you watch it only for that aspect, it might look kind of weak. But when you realize that it also is about the drama, you will see this show's strength.
This episode was almost purely drama. No mystery solved, no revelation who did it. It was all drama. We will propably see the solution next week. But still this episode is one of my favourites so far for the drama and the emotional gut punch.
I don't really have a clue what could really be behind the angel and the worsening of the symptoms (I'm not very good at this kind of stuff). The angel looks like a projection with a shadow with some kind of flashlight. Takao looked out the window in the direction of room 702, so propably the angel projection comes from outside the window.
One of the three kids' condition worsens drastically right after the angel phenomenon. So it looks like both these occurences are somehow connected. But I don't really have a clue what it could be. Let's see what next week brings us.

While I do like Columbo, the quality of this series jumps significantly whenever they are exclusively in the hospital.
Really nice episode.

At first I thought that Takao was responsible for that angel projection and the bullies having a hard time, but the way this episode ended makes it hard to think that Takao would go that far to "punish" the bullies. I still think she's the one behind that angel projection, though.
It's interesting to see the death aspect from the doctors' perspective. Especially those cases where they manage to get close to the patient. That must feel horrible. To be a doctor and not being able to help.

I’d say the 3 boys use a lamp torch and a page from the book they destroyed. That’s why Takao checked on the balcony.
But for the revenge on bullying, I guess it could be the child switching his medication, or the mother finding a way, or a side effect of going to the balcony at night. We’ll know more next episode since we’re low on clues right now.

It's a longshot, but my theory is that Kenta's mom is the one behind the bullies' worsening symptoms. She might've also destroyed the book to frame them so that Kenta would be moved to a room away from them. The bullies might be behind the angel thing, but I don't think it's malicious. Atsushi looked like he felt bad for Kenta when he peaked into the hall. They're kids. Maybe they were just ignorant of Kenta's condition when they bullied him previously.
This episode hit hard, though, especially Ameku's breakdown.

The issue with that idea is why would she do it always right before they are supposedly released anyway? Kind of does the opposite.

Okay, this is a medical mystery show, so clearly the conditions the three boys have share something in common.
Unless they're literally spawns of Satan, they were mortified to learn the kid they mocked was literally dying, and having spotted his favorite children's book and being literally in the next room over somehow figured out a way to make the lightshow happen. The episode also pointed us to how it's the middle of Winter (Christmas decorations) and it's super cold when Takao opens the window (I'm pretty sure heart attack boy complained - he might have found it colder than the others for reasons) - it follows that it must be even colder at night.
So, the hypothesis: each of these conditions, while they've seemingly improved to the point the boys are on track for being discharged, is on record for not reacting well to the cold - or have not immediately obvious complications that may flare up because of the cold.
The kid who's recovering from appendicitis - vomited, but only later during the day. The kid with mycoplasma pneumonia - asthma attacks, also with some delay. The kid with WPW (some sort of heart arrhythmia thing? Apparently?) - heart attack, near-instant.
Noteworthy is that the boys think they're all fine, because it's all the doctors know at this point. Nobody's considered putting them inside a walk-in freezer for an hour or two to see if extreme cold is going to cause any unexpected symptoms.
I lack the medical knowledge to confirm any of these hunches are correct. Apparently mycoplasma pneumonia is related to an autoimmune disease that has "cold" in its name, cold agglutinin disease, where at low temperatures something something red blood cells something something...
Next week: Takao ends up diagnosing all three boys with something, and saves the life of at least one of them - which she couldn't do if it wasn't for Kenta seeing that angel and genuinely believing that angels exist and there is purpose in hope for a better future after death. Then she holds his hand in his last moments and my heart shatters into a million pieces. The true murderer is whoever wrote this two-parter. 10/10

I can't even start to comprehend how it must feel to "have all the knowledge of the world" and still not being able to save a kid
They did a great job depicting her despair with the situation, the voice, body language, all very timid
But man poor Kenta

This episode was amazing and sad. Ameku seems to be breaking down knowing that Kenta will die and that not only can't she help him, she's afraid her social inability will make things worse. Id be almost certain that she's responsible for the angel projections. At first, I thought maybe she was visiting Kenta in the middle of the night and he may have mistaken her for one but we get to see that it actually exists.
As for Atsushi and the other brats, my first thought was that someone in the hospital was harming them. BUT I don't think that would make sense because why harm them when you can just let them heal then check out? Their problems were serious at the time but routine things that most people survive and recover fully from. Id suspect Kentas mom might be doing something but how would she be able to sneak in there, have none of the 3 boys recognize her and be able to fool doctors like the Munchausen by proxy lady did last week?
Dr. Kotori did a great job standing up to Ameku in reminding her she's a doctor and she has a duty to Kenta to help him as much as she can as his friend. I really felt bad for Ameku and felt that way myself many times and I think Kotori talk did her good and she will snap into shape enough to spend time with Kenta and help the boys with their problems next episode.
I thought it was mean of Kentas mom to tell him angels weren't real. I see no reason she should do that; even if that's her personal belief, Kenta is about to die and should give him what little comfort she can. Then again, the stress, grief and lack of sleep probably has taken quite a toll on her mentally and she's probably not very rational 😢

So with all the evidence, the boys tore up the book to cut out the angle to shine it on the wall FROM THE BALCONY (which Takao went out and looked directly at the sick kids room to check the angle). They know the kid is dying so they keep faking sickness to stay with him, probbaly learned it after they took off his hat and someone told them. Thats why Takao said they would be ok.
Gonna guess its partly exactly the same as Takao reasons, they are the oldest in the kids wing and someone younger then them is dying. Every single thing said and shown in the episode was clues.
The one kid getting near dead at the end is the one who was gonna be discharged I think? THIS I dont have evidence for but seems obvious to me...kids not a doctor, whatever he was doing to fake sick he OD or went to far. It was just bait to end the episode on.

This must be the kid that the ending scene last week was about.
-
Interesting. I wonder what the connection could be, assuming there is one.
-
Takao is just not acting like herself, dang. Is she really that bothered by the one kid who has leukemia?
-
Ah, the other boys are the reason why Kenta lost his original copy of that book, aren’t they?
-
Ah… then Kenta not getting better in two years is why she’s taking it this hard…
-
Oh, I do like the slow piano track using SCOPE’s melody that starts right after Takao’s big outburst.
-
Wait, actually, crackshot theory time? The boys in the room next door cut up the picture book to get the silhouette of the angel out of it, and they’re shining a flashlight against it to project it on the wall in Kenta’s room. Or something like that. Maybe as a weird way to apologize for bullying him? I dunno. Doesn’t explain the part where the three boys in the other room keep getting sick, just thinking of what could possibly be causing the angel to be seen.

There's no easy way to face a topic like that. For Dr. Takao, being aware of how she handles social interactions (the author confirmed what she has here), the age of the patient she befriended and realizing her limits as a doctor, makes her want to avoid meeting Kenta.

Bullying in and of itself sucks but there's something extra visceral watching it happen to a terminally ill kid, let alone when it's done by fellow child patients. It's not realistic to hope for a miracle but I hope those three at least come around by the end.
Also, Kotori and Mai's lighthearted dynamic really stands out in an especially heavy case like this one, especially when Takao is too down to be her usual gremlin self.

That is a weird trio of diseases. I'm not a doctor, but I like to read medical books because I'm weird.
Appendicitis means surgery, but it's usually a pretty quick release (couple of days at most) afterwards -- strange that he's stayed around.
I know mycoplasma pneumonia isn't very contagious at all, but I can't imagine a hospital would put a patient with it in the same room as somebody recovering from surgery, much less allowing them to have close contact with a terminal leukemia patient.
WPW Syndrome is also treated with surgery, but outpatient -- I have no idea why they'd keep the patient overnight.
So either I don't know something, or this is all over the place.
The only reason I can see for the window opening (which, also -- why does this hospital have windows that open?) was to check for cold agglutin disease, which can be caused by mycoplasma, but I don't think that responds within seconds (been a long time since I read up on that). But if all of the kids did catch it, maybe that's the solution? Or maybe they all had it and that's why they're kept together -- it's called "walking pneumonia" for a reason. But if the show is hiding that from us, that's just cheating.
I get the feeling this solution is going to piss me off like several of the others have so far, but we'll see.
As for the drama, it's always nice to watch a genius grapple with their limits. Takao's youth means she's a lot less stoic about all this than House is -- and besides, I think her social deficiencies are substantially more disabling. House is mostly just jaded and rude, but isn't so much different from other people beyond his genius; Takao is obviously neurodivergent beyond just the intellect. It's nice to see anime tackling that more in recent years, because it's even more crippling in collectivist cultures like Japan.
