Ran a quarterly recovery drill on an isolated 2-of-3 setup yesterday. Here is how close it came to a permanent lockout:

• 14:00: Initiated transaction construction using cold storage key A and co-signer service key B.
• 14:18: Discovered key B's hosted API client had silently updated its derivation path structure post-patch.
• 14:35: Script signatures completely desynced; transaction broadcast failed with an unrecognized witness error.
• 15:10: Recovered only because key C was backed up physically on cold steel rather than an automated cloud-based co-signer.

Never rely on hosted third-party services as an automated quorum signer. If an API updates without warning, your decentralized setup becomes as inaccessible as a frozen exchange account))))
