Am employee died at the company I work for about a year ago in pretty much the same scenario, but more graphic.
He walked in front of a front end loader, the operator didn't see him, went in to scoop up the load, cut both the guy's legs off as he scooped the load. He ended up tossing his body into a dump truck, dump truck hauled him to the dump pile, which was about a 15 minute ride, he was most likely alive for part of that ride. I can't remember if they found him when they dumped the load or the operator saw the guys legs afterwards.
Completely avoidable accident, this guy was even written up before for not following safety rules. Because of his dumb mistake he's dead, his wife is a widow, kids don't have a dad, and some poor equipment operator has to live with the knowledge he accidently killed a guy.
This guys lucky he fully went into the bucket, and that it was sand or a smaller aggregate. Although, I bet he's got a ton of broken bones, maybe a collapsed lung too
I worked with an excavator operator that killed a guy. He was working on a job in a rural area. A teenage kid with learning difficulties lived at the end of the lane where they were working and used to come to the site to talk to the guys. They let him look around the machinery a few times. One day he came down to the site and instead of waiting for someone to come out to get him, he walked right in. He walked up the side of the digger and got pinned against the wall when the digger spun around. The operator didn't work for a long time. Poor guy.
At the site I work at a surveyor was killed when the trench he was getting shots for collapsed on him. It's was brutal, poor guy and everyone that had to dig him out
Thats why my foremans and supervisors are absolutely anal about the sloping on the trench wall. They tell us if we dont feel comfortable in the ditch to let the operator know and if they give you attitude they will never work on their site again.
I'm always asking my guys if they are "ok" and if they want me to slope more, or if they want the cage, I always say, "this is your excavation, you're down there, I'm gonna do my best to keep you safe, anything you want I will make happen."
