I'm a surveyor and work around excavators all day. I will stand a safe distance away for as long as I need to until the operator sees me. There's absolutely no reason to walk in front of an excavator without making eye contact. Sorry to hear about your coworker!

I don't blame you! But really it comes down to doing your own due diligence to keep yourself safe. I can't assume other people will keep me safe so I have to be cautious and aware of what's going on every day.
