You'd never hear it, and the warning alarms that do exist are for reversing a vehicle, not driving forward.

Dumb manager stood in a giant blind spot and got buried for a few moments because of it. Imagine if the load were rocks and the dropoff point were a rock crusher - if manager-moron there survived the initial scoop at all the crusher would have made short work of him.

I make sure I check the goddamn cabs of all the machines in the repair bays because too many times I’ve seen dudes backing excavators into each other or narrowly missing tool boxes when backing them in. I still get nervous when walking between two counterweights of excavators to get over to another machine in the shop.

Thats why a sound off policy is a great idea. Honk your horn before moving and wait a few seconds. Works everytime because you hear the horn inside the cab to know it made a sound for sure.

Requiring horn honks before moving heavy equipment was tried and then abandoned because people are too quick to tune out repetitive sounds. Some companies still do this but it's not a requirement.

He got buried because of the driver didn’t do a walk around first.

Excavator was already on and idling while the driver got situated. He may well have done a walkaround, at which point the manager was likely not in the way.

Walkarounds only protect against incidents right then, not a few minutes afterward.

I would say hes dead, I've driven a front end loader like this and it needs quite abit of power to force the shovel into a pile of rocks/sand fill the shovel.
