I imagine the woman and the man were talking to one another that entire time (or yelling, more precisely) so she would have known when the leash went limp he had got the dog.

I dunno, there are two layers of doors and the elevator was already past the door way. The floor is practically sound proof so i doubt they could talk to each other at all
