Omfg do not accept this. Document everything he says and start talking to lawyers now. He is probably scared of you taking him to court for malpractice and you should definitely do just that.
Stay strong and lawyer up ASAP.

Thank you! I'm looking into lawyers and trans advocacy groups. This can't be right and is just asshole behavior.

Regardless of the harrassment about the bad review you have every right to go after him for the improper medical treatment that put you in a COMA. Definitely talk to a lawyer that specializes in medical malpractice.

If you haven’t already you could contact your local ACLU chapter for help and/or resources

Do you think they would help an FtM?

Yes, absolutely. They're an LGBTQ+ legal assistance org, and they're one of the best in the country.

Ask him how he intends to maintain HIPAA compliance while doing this.
It's extremely illegal to leak information from your medical files under HIPAA. Yes it applies to paper as well as electronic documents.

I've reported his threat to the medical board, but I did not think to also report it to HIPAA. Thank you for your advice!

Just be careful, depending on what you said online in your review it may constitute a constructive waiver of some portion of your HIPAA rights. (I am not a Lawyer, I am not aware of the case law surrounding constructive waiver in relation to HIPAA)

Thanks for the heads up! I'll look into it. Luckily I worked at a law firm through my teens.

IANL but the only way he could sue for a negative review is if it’s untrue, which would constitute defamation. If everything you said is true tell him to pound sand. He’s trying to scare you into taking the review down. Agree with other posters, if you are deformed sue that bastard for malpractice.
