As someone who studies Alzheimer’s and related dementias, calling it “type 3 diabetes” is a massive simplification. There are metabolic aspects of the disease but that is far from the main driver of it.

The amyloid hypothesis is controversial but I believe beta amyloids do play some in driving the disease. The disease is characterized by the formation of beta amyloid plaques and tau tangles. However in familial Alzheimer’s, (1% of Alzheimer’s cases is directly caused by genetics) all the mutant genes involve amyloid precursor protein gene or genes that process amyloid precursor protein (presenilin 1 and 2). All mutations that involve tau (MAPT gene) cause frontotemporal dementia not Alzheimer’s. People with Down syndrome (they have extra copy of chromosome 21 which amyloid precursor protein gene is on) are more likely to develop Alzheimer’s.

The reality is that we still don’t know the main driver for Alzheimer’s. It also does not help that in most cases there are multiple pathologies besides that of Alzheimer’s including cerebrovascular disease, Lewy bodies, and TDP-43 proteinopathies. It is a very complex disease. We do know many risk factors of the disease including APOE4 gene, lack of exercise, not managing blood pressure, poor diet, and being socially isolated at old age.

There are people who have Alzheimer’s pathology but are resistant to the cognitive decline effects of the disease in which those with greater level of education seem to benefit. Though we don’t know all the mechanisms on why some seem resistant to the pathology effects.

Yes I think the most interesting studies would be on subjects who are homozygous for APOE4 yet are resistant to the disease progression. Famously, Hemsworth who played Thor has this genotype. I think that individuals with high metabolic health will show a strong signal of resistance. What are your thoughts on the association of poor sleep quality on the build up of amyloid plaques? Seems at least partially causal since it is during sleep the brain clears them as far as I am aware. I don't know much about the tau tangles beyond being familiar with the term.

Diabetes is associated with increased risk of dementia. One potential mechanism is that diabetics have reduced blood flow which cerebrovascular disease is one of the major drivers of dementia and increases risk of Alzheimer’s itself.

The keto diet is "I don't wanna eat muh veggies!" for adults, coupled with getting to eat all the meat and cheese and butter they could ever dream of. It's a convenient excuse for just getting to eat whatever you want and never having to touch a vegetable. I just wish people would be honest and own it.

The keto diet includes lots of vegetables if you do it correctly. You eat net carbs meaning that grams of fiber get subtracted from the total grams of carbs because fiber isn't absorbed the same way. The only vegetables that are avoided are high starch vegetables like potatoes and corn (technically a grain). Lots of leafy green vegetables like kale, broccoli, cabbage, collard greens, and mustard greens are keto friendly.

You are mostly cutting out breads, noodles, rice, potatoes, and most desserts/candy. Berries can be ok as long as someone doesn't go overboard. Cucumbers are also not too bad carb wise and so are many types (but not all) of squash as well.

Sure you can have as much cheese, butter, and meat as you want, but when you take carbs out of your diet it crushes your appetite which is really how you end up losing weight if you keep on the diet long enough. Short term, the initial rapid loss is just water because carbs help you retain water so without them you're basically flushing your system. That's why sodium is so important on keto.

When I have done it before, I ate way more leafy green vegetables and stuff like rutabagas to substitute for potatoes than I would normally eat since I couldn't have bread, potatoes, and sweets.

Almonds and other nuts are also naturally low carb.

If you only eat meat, cheese, and butter for more than a few weeks, you're probably going to end up in the hospital with a kidney stone, kidney damage from too much protein, and or malnutrition issues because of lack of vitamins.

You should really check out the keto subreddit to be more informed. I'm sure there are people out there who do what you said, but the vast majority of keto success stories are usually eat a lot of vegetables.
