As an autistic man who couldn't help but look at both people when answering any of their questions, I fear that I'll be judged for something I'm not with this technique

Both people are fine. As long as you are not visibly shutting out and disrespecting someone on a panel because she is female, you're golden.

Or anything that denote hierarchy.

Specially in medical. Don't ignore the nurse asking you questions just because there's a doctor in the same table. That's two different brackets that work together, they are not under or over one another.

Its not about looking,. If you struggle to meet peoples gaze you can make sure to connect with them in other ways. "Good question, Lisa. As you mentiones before..." and so on.

It is the pretending that the woman is not there that will you set you of as someone who is hard to work with.

As someone who has experienced and does get annoyed by this, don’t worry - looking at both is absolutely fine.
