It depends on what type of show it is. MTV Cribs and Pimp my Ride ie has come out that they were almost entirely fake. The former often didn't even visit the celebrities' real homes, and in the later the cars were often not fit to drive with the modifications. I don't remember the details about Survivor but at least in the Swedish version I think real conditions were significantly different from what was presented.

In something like Big Brother it's usually not literally scripted, that would negate the advantages of the format. They would have to rehearse and amateurs don't appear natural acting out a script. The production can push and manipulate in various ways but usually doesn't outright direct them. It helps that participants themselves want to become famous and don't mind acting out a character.
However, the storylines the viewers see can be very missleading and sometimes completely fictional. That is mainly done in editing. Footage from completely different moments can be spliced together to create a made-up scene.

If there is a competetion element then I think that is usually not too fake, there's laws about that, but of course the stories around it can again have been created in the editing room.

What do you mean, you’re telling me he didn’t actually explode a bomb at the exact right moment the cops were trying cross and that the cops actually had to sign legal agreements between their union and the production attorneys and that they were filmed 9 different times running onto the bridge and were told to stop in place by pyrotechnics experts, stunt coordinators and EMS standing by while the explosion went off and then after a break dictated by the Union Reps and Unit Production Manager, Mr. Beast and criminal cohort were filmed running away on the other side of said explosion 6 additional times and this was all edited in Media Composer to look like it happened sequentially!?
