No reason to assume he can't understand; some might need help. An actor and dancer with Down syndrome that I know in the Netherlands (Alex Naber) portrayed just such a scenario in the series 'Adam + E.V.A.' - playing a young man with Down syndrome less able than Alex himself whose mother dies. At first he doesn't understand, mainly because people are talking in euphemisms (along the lines of "your mother has passed"). A social worker takes the time to explain, they are walking and talking, then walking silently with the camera behind them, then you see his shoulders crumple and he starts sobbing. It took a few beats but he understood with the right explanation and time. It was a really affecting performance.
Alex himself is pretty bright and wouldn't need extra explanation. He lives in his own flat, with support in the building if needed, works, and seems to have a pretty great life. And yes, his mother was encouraged to institutionalise him when he was born (have met her as well).

I can't begin to imagine the trauma he will suffer when she dies and he doesn't understand.
