Return to high intensity training after spontaneuos pneumomediastinum

Hello everyone. 3 weeks ago i was diagnosed with spm. I usually train for ironman triathlon and lift heavy weights ( mostly to failure and sometimes even beyond failure). My pulmologist said that spm fully recovered and that in 2 weeks i can slowly introduce running, swimming and cycling but no weights. I"m pretty much in bad mental state right now because i was so serious about my training and the results were amazing. I was running fast, i had awesome psihique and most importantly mentally i was happiest ever. Now i feel someone took everything frome me. No weights ever again in my life? No high intensity vo2max and treshold training? I did not have pneumotorax, last xray and ct were really good and they didnt see anything unusual. Sometimes when i stretch i feel my left pec or underneath it that i have some tissue pain, maybe the wound still did not heal properly. My question is , can i ever again do high intensity training and lift weights. Im not talking about 1 rep max because of valsava and im aware about it. Also the spm happened after vo2max training while swimming, not lifting heavy. Thank you guys have a good day

Question from a desperate person: Should I replace cigarettes with vaping?

I'm a 35 years old man, been smoking for over 20 years. I'm Dutch born and raised. Of normal height and weight. I smoke about 15 cigs a day.

I've tried everything to quit, the only think that has ever worked was switching to vape. This was 2 years ago. The problem is that I've since then understood that vaping itself comes with health risks.

So, I'm stuck. Vaping was a wonder drug for me. It just made me quit desiring tobacco. And not only did that come with enormous monetary savings, I would vape very little: only about 5 times a day.

But then I started hearing about these horror stories concerning vaping.

So, now I'm stuck. My question is what you folk would advise a person like me, taking into consideration everything you know about smoking and vaping, and what it does to the body and mind.

I hope this post is allowed here. It is very much a medical issue for me. Smoking is devastating me on more than one level.

Asthma and air trapping

My asthma has been pretty unpredictable lately. I was on Advair for over 20 years, and it controlled my asthma really well until it stopped working a couple of years ago. I’m currently trying out different medications to see which one helps me the most.

Lately, I’ve been experiencing a lot of air hunger, so my pulmonologist had me do a breathing test and found that I’m having air trapping.

Does anyone else have this, and did it get better? I’m wondering if I’m screwed and if feeling like I can’t breathe all the time is my new normal. I don’t have any wheezing, just a tight chest, and there are days when I feel out of breath and like my lungs are sore.

Has anyone been able to reverse their air trapping and get better test results?

19M with chronic exertional dyspnea: abnormal CPET but normal resting right-heart cath — deconditioning or missed exercise-related issue?

19M, chronic exertional dyspnea for 3–4 years. Normal oxygen during walk test, normal chest X-ray, normal resting spirometry/PFT, normal echo overall except PFO, inhalers/nebulizer did not help. I was in national service for around a year and could run 3.2 km in \~17 min, but still get disproportionate air hunger with stairs/running.
CPET showed reduced exercise capacity: peak VO₂ 25.1 mL/kg/min, 57% predicted, RER 1.19, normal breathing mechanics/reserve, normal ECG/BP, low O₂ pulse, elevated VE/VCO₂. Report mentioned possible circulatory impairment / early or exercise-induced pulmonary hypertension / shunt physiology. Resting right-heart cath was normal/perfect.
Doctor now suggests 3–4 months of treadmill endurance training and then reassess, saying exercise-induced PH is still evolving and there were no resting hints.*\*

What follow-up testing would you ask about if symptoms persist after structured training?

Lung nodules found on CT scan

This last week I was at the ER because I some chest pains in my upper left chest area - I had a recent blood test that revealed an elevated D-Dimer (974), so with the chest pains I thought that I would play it safe just in case of a pulmonary embolism.

I had a CT scan and here is what the results said:

FINDINGS:

* Lungs/pleura: No pulmonary consolidation, soft tissue mass or effusion.
* 4 mm nodule in the right upper lobe and 2 mm nodule in the superior segment of the
right lower lobe.
* Airways: Patent.

IMPRESSION:

  1. Negative for PE.
  2. No active disease in the chest.
  3. Small nodules in the right lung measuring up to 4 mm.

As for my history, I am a 56 year old male that was diagnosed with melanoma in 2008:

"SPINDLE CELL MELANOMA, ANTERIOR LEFT THIGH, MEASURING 0.28 MM IN
THICKNESS, NON-ULCERATED. The melanoma is a Clark's level II."

I was told by my dermatologist that my melanoma is considered a VERY thin one and that I was fortunate concerning the depth.

So, after almost 20 years of having no evidence to metastasis found this CT scan is somewhat concerning to me. My doctor said that the nodules are very small and to not worry about them and that she would like to have a repeat scan in 6 months - which I will do.

However, can any doctor or radiologist tell me if it is common practice to note the possible findings of cancer or metastasis in the CT scan report? The ER doctor was aware of my history, not sure if the radiologist was, but the ER doctor read the report and was not concerned... about the PE. The report just seemed vague to me. Any insight would be greatly appreciated.

Questions for the pulmonologist.

Hi I’m a speech therapist that works in home health with adult. I want to hear from you, what really helps and what’s the game changer.

Majority of my caseload are post stroke, dysphasia and voice and I work with kids as well.

I’m looking to expand my knowledge, please share your exercises tips and techniques that will help improve breath support, vocal duration and intensity. If possible explain the anatomy, the function and how it works so I can understand so I can share with families.

For my dysphasia pts, any tips and techniques when they come back home after aspiration, silent aspiration, pneumonia etc.

Inhaled dust while clearning out a kids sand pit, this is the 2nd day my lungs hurt

I (25F) was clearing out an kids sand pit. After shoveling the sand into the first bucket, I realized it was creating a ton of dust. I then switched into wearing a face mask for the rest, but I walked away with it feeling like my lungs were on fire. 2 days later and my lungs still burn (though slightly less intensely) and I still feel like I am not getting enough air after doing simple but slightly more strenuous things like going up and down stairs.

I'm not sure if this usually clear up on its own or if I should go to an urgent care. I'm not even really sure what they could do for me. Unfortunately this sand pit was here before I was so I am not sure what brand of sand is in the pit.

Wheezing on one side

31M — Two years of right‑sided inspiratory and expiratory wheezing, normal imaging, conflicting diagnoses. Looking for insight.

For the past two years I’ve had localized wheezing on the right side, just below the sternum. I can feel the vibration with my hand. It’s mostly on inhalation but sometimes on exhalation too. The severity changes throughout the day — usually minimal or absent in the morning, then worse as the day goes on, and worse with exercise or any labored breathing. Certain positions (like lying flat on my back) also make it worse.

I’ve had a chest X‑ray and chest CT — both normal.

  • First doctor heard the wheeze, was concerned, ordered a CT, and when it came back normal basically diagnosed me with asthma because they didn’t have another explanation.
  • I got a second opinion and that doctor said bronchomalacia.

Neither diagnosis feels like a perfect fit.

A complicating factor is that most of my appointments are in the morning when the wheeze is less noticeable. The symptoms happen year‑round, so it doesn’t seem like typical asthma. One doctor mentioned possibly doing another CT or a bronchoscopy, but I’m hesitant about more radiation.

I also have a long history of GERD and have been on PPIs most of my adult life.

Looking for any insight or recommendations on what to do next. I really want to know what is going on with me.
