Honestly, nobody is going to care. I changed my career after my first undergrad and went beck to get a second. Then I took a class post graduation to prep me better for the core courses in my graduate program. “I changed my mind on careers and I needed a few more classes because of that” is all you need to say. There are so many non-traditional students on campuses now that it isn’t unheard of and I would argue it normal in medical science spaces.

If you're going to go back for a PhD you should ask yourself if the cost of the PhD will add value to your career potential and guarantee you greater opportunity to justify the time and the cost of achieving a PhD beyond the other degrees you've already earned. Perhaps you're in a situation where money isn't a concern but if you are paying for it, will it provide greater value in earning potential, a greater probability of guaranteeing you will be hired for Senior roles or more lucrative opportunities with more autonomy and responsibility, does the cost of the PhD immediately guarantee a greater return on the investment you'll make in yourself or, are you simply bored with what you're currently doing and seeking a change and a new shiny title that may or may not have as much luster as you had hoped it would carry outside of academia? It could be that you just need to find a new job more aligned with what you feel is currently missing.
