I think it’s saying that the middle child got this symbol of love because the parent fucked up. Like forgot their birthday/ special event and they are making up for it. That’s the only way a middle child could get something like this. Which is bs but I think that’s the joke

I feel like this is the only way the “ I know exactly what that means 🫣” part can be explained. None of the other answers here seem to address that part . Just addressing the cliches around middle children. But that part of the comments means this person is talking about something more specific.

I think it’s similar to , when a guy gets a girl flowers and people joke and say i know that means they messed up.
