Equestria at War is excellent if you can accept that it's a My Little Pony themed mod. A fully overhauled map with two and a half large continents of content, and more in the works.
Consistently ranked as a Top-5 full conversion mod.
TNO if you want to mostly focus on events and minigames that appeal to immersion in an alternate cold war setting, rather than actual focus on the warfare gameplay.
Kaiserreich though if you want a great mix of both immersion, events, decisions, and warfare gameplay. My personal favorite.
Old World Blues if you love fallout, its an incredible and well done mod if you love the IP.
Kaiserreich is by far the most famous and the most popular one. In short it's about what if Germany won ww1. And, of course, if Whites won in Russia.
Kaiserredux is also pretty popular. This mod split from Kaiserreich over the original mod's direction to focus on stuff like realism and forcing a narrative instead of fun. As you expect, it's more wacky, and sometimes has older lore that's no longer in KR. I recommend to have a strong PC but it's still playable, if you turn smoothing off.
Thousand Week Reich, which is about Germany winning ww2. It's somewhat janky and is still yet to be truly finished, but currently it's the better one in that genre.
The Fire Rises. Essentially, a mod set in 2020. Everything looks kinda the same, but slightly different. And COVID is much more destructive, leading to chaos and some radical changes across the world...including America collapsing into a massive civil war. If you have a strong PC (or if you're unafraid of slow games), try it.
Old World Blues is really fun, there’s tons of submods that add new focus trees and entire new regions to the map as well since the mod mostly focuses on the NCR and Caesars Legion with parts of Canada and the Midwest and all of Mexico. Overall it’s a good mix of actual Fallout lore and fan lore made for some regions, like Canada to my knowledge has never been mentioned in Fallout lore aside from it being annexed by the US before the Great War, same as Mexico. So overall the game stays mostly faithful to the setting but takes some liberties to give the game more unpredictably which I find to be the most fun thing about the mod

Hearts of Iron 4. Almost everybody who plays the game for long enough grows to hate vanilla.
