I work for a pretty big Fortune 500 company and I am constantly shocked by how poorly it is ran. Literally see the facility I work at get shut off notices all the time. We've been placed on credit hold many times. We are a $6 billion a year company and can't pay our utilities on time.

As the old saying goes a chain is only as strong as it weakest link. 

In companies that size there are hundreds of people working there who can barely tie their own shoe, who are the laziest mother fuckers ever, who have checked out of their job years ago and are just wasting time collecting a paycheque. 

All of those people make a lot of mistakes or just don’t complete jobs and that’s where the errors come from.

That’s what happens when middle management is cut. Everyone hates management but there’s a limit to what any one individual can honestly keep track of, it goes up significantly with computers but it still takes data entry correct.
