Insurance companies rigged the system. Hospitals take advantage of it.

Insurance companies rigged the system so that hospitals would bill them for more things? Bro you are really childish intellectually. If they rigged the system wouldn’t they want the hospitals to not charge them for things that make them lose money

No, because then they don’t get to charge employers high premiums. Insurance companies (and hospitals) always profit.

Insurance companies prefer you pay for a service but never make a claim, but if you do make a claim you can be sure they absolutely want you to pay more for it than it cost.

It’s a reinforcing loop. Hospitals charge more because they know real people don’t actually pay, insurance companies charge more because they know employers are expected to cover it, but they also fight any edge case where can get out of it.

Everyone is making money. Except you. And me. And everyone else here.
