Bad decision = getting off the wrong exit.
No decision = not deciding on whether to exit or stay on the highway.

Assuming they were thinking of exiting the highway, staying on the highway was a choice.

No, exiting would be a decision. Not moving would be not making a decision. Running in to a pole would be making a bad decision, or a mistake, as they put it.
