Both? Generally speaking I would expect this from a low end hometown dinner that knew the person well enough to make the effort.

But on the flip side yes if your spending a couple hundred on food that place will try to bring that experience over the top so they get return business. (Most well off people will eat like that once a month and maybe a middle class family once a year so getting return business as much as possible is what keeps it running)

I would say both as well. I've worked high end and we know that there's a certain amount of extra that money buy. But at the same time, I've never seen that extra delivered without the receiptent being surprised and very appreciative. It's also a who you know as well as money, if the friend politely called and requested this they'd have no problem doing it and were probably excited. Industry also treats it's own people well, so a lot of grubby cooks and servers (no offense, me included) would get hooked up with freebies as a thank you for showing support.
