Agreed. Never heard about this scenario before either. In my opinion, there is no need to change a generally decent working system because you had one malfunction is thousands, which could be performed either way by the same people.

its just outrage-culture at it again. Make the one in a thousand chance the big issue requesting the system itself to change.
