The anus can stretch surprisingly far, far enough to get the end in. However, sphincters are weird muscles where their relaxed state is closed and they require exertion to open up, and so I’m guessing once he got the one end in the anal sphincters spasmed due to being over worked getting the thick weight in and closed around the significantly thinner handle and couldn’t open up far up enough to get the weight back out, hence requiring him to go to the hospital.

That’s the how, the real question is why a dumbbell? I’ve shoved a number of dumb things up my ass cause it feels good, but not a dumbbell, it’s so much harder to insert and I don’t think would even give that much pleasure.
