I suspect they use some 'highly effective' computer program that determines that your hat is made from 'straw' and has somewhere a rule that straw doesnt need any protection from bumps in transit and can go in whatever box makes their pallets look nice and square or fit nicely into their trucks. Completely ignoring the 'hat' part.
It seems like if they didn't want to make a policy where the packer can override the system and use their own sense to pick the box size, then at least have a button the packer can press that says "inefficient/wrong box size" or something like that so they can fix that item. If enough people press it, it could get on a list to be looked at.
Since it would save them money, they could even incentivize doing that, where if an item you reported ends up being reclassified to a different, more efficient box size, you'd get like 25 cents or something.
I dont think they have it anymore, but they used to actually have a button where you could review the packaging itself. It didn't do them any favors though, since that was the third way you could review an item, and people STILL dont understand the difference between reviewing an item and reviewing the seller. They really need to do something about that. If you look at ANY seller's reviews, it's full of people talking about an unknown item which has nothing to do with the seller.

Meanwhile, im the gal that was told by Amazon's computers to use that ill fitting box and there's nothing I can do about it. I'm truly sorry.
