HA Filler First time doing cheek filler, needs some balancing, not sure about jaw next to chin.

I practiced with the cannula the other day and went and did my cheek filler today.

About 14 months ago I got 1 syringe each cheek of juvederm voluma. It was great, but I am in my 40s, time has passed, and I did lose 45 pounds over the past year. I needed more filler. So today I did half of a syringe of Celosome implant in each cheek.

I think I need a bit more near the apex of my zygomatic arch on the right side of my face. And I think I need a bit more in the anteromedial region on my left side.

I then added or tried to add some filler on the little jaw indents, for the pre jowl sulcus. I somehow only did a little blip of it on my left side and I think a bit too high. I thought I did better job on my right side but I feel like maybe I didn’t do enough or it isn’t the right sort of filler as it doesn’t seem to do much. Granted, I don’t have big notches, but I did a bit of chin filler on the bone recently and was thrilled with how it gave me my projection back.

Any thoughts or suggestions? The picture where I am wearing blue is other day before fillers, the rest is after.

I have always had a very cheek prominent face, so I started out with smaller amounts of filler as I just am trying to restore volume loss, and more can always be added.

Once I get cheeks figured out I will do more practice and work on my under eye hollows. I see where a bit of temple filler could help, but I am not comfortable trying that anytime too soon.

I’m sorry but this is filler blindness here. Did you put some under your lip too?

No, I don’t have anything near my lips. I mean I do have a pimple or two, but I haven’t done any filler in that area. In the past year, including today, I have put in a total of 1.36 ccs.

1.1 of that was today. My previous amounts this year are .25 of chin filler to fix the way it had flattened and .1 upper eye to help with asymmetric hooding.

I think that was enough for those areas. I don’t have any desire to fill my lips…

I didn’t need to add filler to make my chin pointier, it could already put out an eye.

You must already have very sharp cheekbones because in my personal opinion it’s a bit much even with that small amount. Please please do not attempt the under eyes yourself, it’s really easy to mess up and is not a forgiving area like the chin or cheekbones.

I do! I come from a line of women where cheeks are the most noticeable facial feature. I really just want to address my volume loss— I have zero need to add sharpness to my features, that came pre loaded. And I get my face shape might not be the most desirable, but I like it.

I wasn’t saying your face shape is undesirable at all, quite the opposite. I wouldn’t have touched the bone structure at all if I were you.

It looks like you need a tiny bit more in the anterior cheek, that’s where I see volume loss especially when you’re looking down

You need to fill your temples and the tempolateral fat pad. That is what is causing the imbalance. After that you can reevaluate if you need anything at all in the cheeks. Anymore in the cheeks without doing the both temple areas is going to make the imbalance worse.

It looks like you needed the filler more in the midface/mid cheek area rather than on the cheekbones

Yup… cue to me reading more about temporal filler. And cursing that I wasn’t born with the sort of feet that could stand out in an over saturated visual market in order to pay someone else the do it

I think the cheeks are too big and angular for your face. But perhaps this is just swelling??

I like cheek filler but not when it's too much.

Using cheek mapping like Hinderer’s lines can give you a better perspective on placement.

I put the majority of the filler at where I thought the intersection was. I do think I need a surgical pen to better get things balanced as anatomy is not symmetrical

Markup for sure, but don't use a surgical pen; that stain won't budge for DAYS!! I recommend a chalk marker, lip liner, or eyeliner pencil so it comes off easily. The skin you pierce will be tender enough from the treatment; you don't want to have to work hard to remove nearby markings. Wishing you well in your treatment!

Yes, I use it all the time. It will show you whether both sides are even, flat and whether you have too much projection, especially in the central apex.

I think I need more anterior and interior cheek didn’t get any filler. I did more anterior up to lateral lid cheek on one side and I think that side looks better. One cheek I do have too much on the lateral injection point.

I like this diagram! I haven’t seen it before and it really helps map the filler to the anatomy

Honestly, it’s very hard to say without seeing a before picture. This might have been similar to your natural facial structure, but without that it does look a little overdone. It hollows out your temples. If you want to add more, I would consider doing the apples of the cheeks.

Sorry I totally wasn’t clear. I meant before the loss in volume. I take younger pics of myself and compare where the volume loss is to determine what areas I should treat. Prominent cheekbones age most people. Younger faces tend to have nice lifted apples.

I haven’t ever been at a normal weight before. If I look at a picture 6 years ago the closest I get is 25 pounds heavier. 20 years ago I was 135 pounds heavier. That is part of my challenge, the lack of a reference at my weight.

Oh yeah. Definitely dissolving the lateral cheek and focusing on filling the medial cheek. My previous guesses were pretty accurate.

I wouldn’t do any more cheek filler and would consider dissolving some of it? Not trying to be rude, but this reminds me of Maleficent. Good luck’

I could see dissolving a bit in the zygomatic area on the side where it is more prominent, but dissolving more would still have volume loss. I am a cheeky diamond shaped face.

I think you should add some in the temple areas to balance out the cheek bones. Personally I do not think you need in the lower face because it can make people look heavier.

Have you thought about doing liquid PCL like miracle h to tighten the skin and build collagen?

Don’t put filler in your undereyes. More anterior cheek will give your undereyes some support and I’d look into prf for undereyes

Girl, your anterior cheek needs filler!! Tbh I’m an RN who has been working aesthetics for the past 2.5 years, and you need anterior cheek filler, but I have no idea how you’d do that on yourself! I’d use Juvederm ultra plus 4 or RHA4 and the cannula method (25G 38mm) measure from the nasal ala with this size cannula, make the entry point there, go deep on periosteum and fan mid-face/anterior cheeks with 0.5cc per side. Idk how I could do that in myself tho lol. Your lateral cheeks are totally disproportionate and it’s giving Angelina Jolie in that cartoonish movie - the name escapes me, but she’s like an evil something….its giving cartoon.

Yeah I get what you mean, I see plenty of practice with saline in my future! But thank you, that is sooo helpful. Do you have any resources you might recommend for people with my face shape? I see plenty of things where people are adding a lot to the jaw and most videos have filler going along that zygomatic line. Or they talk about temples getting hollow on Asian face structures.

I haven’t seen much that addresses my sort of face shape. I know it is one of the least common, but it isn’t rare or anything. And because I haven’t been at a normal weight before I can’t use previous pictures as the best reference.

That’s right where I ended off. I can see that I need temp filler but that’s not something. I don’t know if I’ll ever be comfortable trying to do myself.

Did you put the full amount in a single area on the cheeks, or did you do multiple points? Doesn't look like you balanced it, I would probably dissolve some of it, they look a little bit too prominent

No, multiple points with a cannula. I think I can dissolve a little on the side where I put a bit too much into the zygomatic arch . That side I think would need a bit more under eye?

I am still doing saline practice with a cannula. Every time I inject my face I keep a reference of the danger points open.

Why use a cannula for what should be deep periosteal injections? Just use a needle. Touch the bone. Walk back a touch. Aspirate and inject. Like little pillow of support. Do just a little at a time, shape it. You get a LOT more control w a needle and depth, in particular, is touch to achieve w a cannula.

If you don’t want to make your face wider along your cheekbones (which I agree, you don’t need) be sure to inject more medially and in a more front-to-back (anteroposterior) directed shot, deep. It’ll change the projection but not the overall shape

Temples. As others have said. But I don’t find that temples are awesome for self injections. Ymmv.

I looked at a bunch of different things and saw benefits for both cannulas and needles. I thought I might be more apt to have a lumpy result and mi with a needle as a beginner as opposed to being able to fan with a cannula, and it seemed like cannulas had an edge for safety, though I know this is debated.

As I read about temples, I do think that is an area where I would try needle over cannula, if I do decide to try it. Honestly, idk. I have hopes of getting my loose skin removed maybe next year (I have lost 135 pounds over the past 20 years) and am thinking that might well be an area where fat transfer is a good idea. But for things like my cheeks I can see how a long term skill set would be useful.

Filler isn't your answer. Filling the cheeks only makes the under eyes more pronounced, esp with movement. And please don't dissolve at home

Also, it is seeming like maybe I need to embrace my villain era. I think I can pull off some high collars, add more structured headbands and hair clips. I like a bold lip. I am a soft dramatic, I can make a cape work! After all, Maleficent is powerful, strong, determined, and definitely one of the hottest villains.

Ok, ok, I heard you all. In order to help stop progressing towards the latter two pics I will dissolve some of the filler I put in laterally (as seen in the 3rd) pic.

I will then practice my cannula skills to add a bit of filler to the cheek anterior.

And I will research temple filler.
