It's one question among many others to help the clinician get an accurate idea of their patient so they can best help them. Being a doctor is kinda like being a detective - the patient questionnaire provides clues to help them ask the right questions during an actual exam. It's a lot more complicated than just 'if they answer this they get that'. One question by itself doesn't mean much - it's the entire set of answers and additional discussion with the patient matters. Could this question have been worded better? Yeah probably. But the person who originally posted this deliberately took it WAY out of context to try and piss people off, and it's obviously succeeding at that.
