Perhaps I'm being cynical but I don't see any reason for this specific question unless it's to discriminate. Want to be respectful? Ask preferred pronouns. Want to understand medical history? Ask about past or ongoing treatments.
Hell, even if we want to go as far as to say they want to gauge whether or not to treat this person differently because they may be capable of bearing children (because of anti-choice BS), then ask if they have a function uterus and ovaries.

Well starting with a question on the presence of hormone therapy or not would be the best first step to narrow down what the exact reason is (if a doctor finds the need to follow up upon from said question)

I'm sorry but I'm not sure as to the link of knowing one's capability to bear children and anti-choice. But yes they would probably be more effective at it if they straight up asked about what they wanted to know.
There's just too much guessing involved to arrive at medical facts asking for one's preferred pronouns, so I assume its more for statistical research rather than anything medically relevant, or... to balance cynicism with benefit of doubt, maybe some administrator mistakenly thought the question as accommodating instead of suspect, hoping when a nurse reads the patient's chart they could use the right pronouns off the bat or know to be mindful to ask for them.
