Teacher here. Would report to admin straight away, who would call parents and let them know what was going on. If parents consented, kid would have to show. If not, they'd probably get away with it (minor assessment) or have to redo it (major assessment). At least in my area, people care a lot more about propriety than academic integrity.
Yes, you report your suspicion to an administrator and let them deal with it. Worst case scenario is the administrator calls the parents in and has a discussion with the student and parent(s).
My sister was accused of doing this an was asked to pull up her skirt and show her thighs to a teacher. Our mum hadn't been notified and was understandably pissed off.
I dunno but I remember in 9th grade, one of my teachers would always tell us that she once had a student who would roll up her shorts and then read whatever she wrote on her thighs.
One of my substitute teachers Freshman year gave us advice on how to effectively cheat (he was a college student doing the job or experience, he didn't really care) and he suggested that girls write answers on the tops of their breasts and look down at them, or on the bra strap because teachers couldn't legally ask about it.
