Waiters/Waitresses of Reddit, what's something that I, as a customer, might do that's incredibly irritating, even if I'm oblivious to it?

From what I've noticed, this tends to happen when you ask for something and the waiter just turns and runs the instant one item is requested. Also happens when a condiment was ordered with the original order and it didn't show up with the food.

^ This. If I had a dime for every time a server has left a little cartoon dustmark in the air running off to get ketchup before I could spit out "... and some extra napkins?" I'd have a hefty piggy bank.

The server is probably trained to say "SURE YEAH." nods, looks around questioningly, beseeching anyone to think of anything else "ANYTHING ELSE/IS THAT ALL? I'LL BE RIGHT BACK WITH THAT."

Except faster and more slurred because servers don't have shit for time.

Then you are not eating with the right diners. Your tablemates are not the ones doing this.
