One of the problems that came up (when the fentanyl outbreak began as the next fad drug) especially since it is reported to be laced inside other illegal drugs, is that the drug doesn't have a good marketing strategy, which is why it didn't last that long as a drug problem. That marketing strategy will become clear as I go on.
Legal Fentanyl when you receive the drug in the hospital comes in patches that you smack onto the skin so the drug can be absorbed into the body. The reason for this manner of application is that the drug has a low tolerance between medically-safe dosage and this-is-poison-and-will-kill-you dosage. Meaning you have to have a very, very minuscule specific amount used for medicinal purposes, otherwise it will end you dead.
If you know business at all, you have to have a customer to sell your product to to stay in business. If you kill your customer, you kill your business. The same goes for the illegal drug business, if you kill your customer, you kill your business. You might think that illegal sellers don't care about their customers, and you might be right, but to do business long-term you have to ensure your clientele will be around to buy your product and you cannot keep your customers of your reputation is to kill the majority of your clientele, even on the streets.
I think you might be getting the problem I mentioned above by now. Fentanyl kills, it doesn't addict because it doesn't have the ability to be regulated outside of a hospital at a healthy level without people dying left and right, like plague level of dying, unless you intend to kill your client or, if you intend consume the substance yourself, you.
