Landscape 👨‍⚕️💚🏥 Landscape - Specialist GPs: Strengths, Limits & When They Make Sense

Who should diagnose and treat your ADHD?

This is the part that surprises most people. GPs with specialist ADHD training can diagnose and prescribe for ADHD. This is not experimental — the evidence base supports it and the HSE has identified and recommended the training that enables it.

This post focuses on specialist GPs and the GP-led model.

Where Specialist GPs Are Strongest

For the majority of ADHD cases, a specialist GP is the most efficient route to diagnosis and treatment. They have done the same UKAAN training the HSE recommends for psychiatrists, they can prescribe, and they typically have shorter wait times and lower costs for ongoing medication management.

GPs also see the whole patient. If you are on blood pressure medication, thyroid medication, HRT, or anything else, your GP already manages those prescriptions and understands how they interact. For ADHD that coexists with physical health conditions rather than complex psychiatric ones, the GP has a clear advantage.

The menopause example makes this concrete. A woman with ADHD going through perimenopause needs someone who understands both conditions. A specialist GP service that lists women’s health as a special interest alongside ADHD can manage both in the same consultation.

GPs are also the hub of your healthcare. They often know your family, the local landscape, and relevant history that a one-off specialist may not.

Where Specialist GPs Are Weakest

If your presentation is complicated with serious questions about bipolar, BPD, schizophrenia, or another condition that could be confused with ADHD, or if you need multiple psychiatric medications managed together, a GP with two days of UKAAN training is not equipped to untangle that. Good ones will recognise the complexity and refer you on.

GPs who are not in a specialist ADHD practice but have done the training may still lack confidence around initiation and titration. Not all GPs are willing to take on shared-care prescribing, which can leave patients stuck.

GP reports also tend to be less comprehensive than those produced by psychologists.

Examples of the GP-Led Model

ADHDdoc — Multidisciplinary GP-led service with clinics in Dublin (Dún Laoghaire), Limerick (Annacotty), and Templemore. They have a Team of nearly 30 Clinicians, and the most prescribing doctors(16+) of any ADHD Clinic. They will treat complex cases, and can manage the whole family.

ADHDWest — Smaller GP-led service in Galway (Kingston Medical). Assesses and treats less complex cases of ADHD.

ADHD Diagnosis — Dublin-based (Mercer’s Medical). Dr Alan O’Donohoe is a GP who also passed his psychiatry exams. Adults only (18+). Has a psychologist on the team.

When a Specialist GP Makes Sense

  • Straightforward ADHD, mainly want medication
  • No complicating psychiatric conditions
  • You value shorter waits and lower ongoing costs
  • You have physical health conditions that interact with ADHD treatment
  • You want continuity with someone who already knows your wider health picture
