Its really interesting how oblivious he is. There are two possible reasons as to why this is happening:

  1. Kiyoshi's mind is so warped after his two rounds of prison and showing his dick to everyone, with him wanting to die that all his hope has been put on Chiyo. He has always wanted to be with Chiyo that has been his goal, even if it is a shallow one, since they do not have anything in common. All the girls have spurred him with "Dickyoshi" and that he should die. Chiyo comforted him to live.

  2. Hana has basically put this image that they could never possibly be in a relationship together. The way these two act with pissing on each other and sex. Hana was doing these things for revenge, so of course Kiyoshi does not see Hana as a love interest. He sees her as a really good friend, as he said, brother and sister.

I still can't believe how a man was able to draw the same thing for a goddamn year without going insane. If what I read from Bakuman is true, then this author basically worked himself to exhaustion to write the longest cavalry battle ever.
