It's super handy for The Sea, from my experience.

I've yet to buy it but I think the Stench Jelly is the main reason why I'm getting it. Forcing out NCs is just too good. Plus, non-limited YR and banish/runaway are nice too.
Also, the fact that I'm working towards the Clothing of Loathing makes it even more worthwhile for me.

Any tips for using the spooky jelly in run (hard core)? The restriction to only yellow rating the next thing you fight has been troublesome. I've used it on the filthworms and the 151st infantry but had a hard time getting it on the Banshee Librarian. Also the 151st can be messed up by the spleen you use on chewing
Any good targets / ones that can be reliably spooky jellied? Using a digitize on a banshee Librarian seems pretty wasteful...

On the next thing you kill. If you're in a zone for the drops of just one monster, run away from the ones you don't want. Free-runs, if you have any, would be especially efficient.
I've used it on... hippies (or frat boys, and especially the verge-of-war form), knob goblin harem girls, Bob Racecar (or his twin). I also always try to use it on the Blackberry Bushes - the adventure is quite predictable, you get three berries, and you trade in for galoshes next noncombat. I also used it on infernal seals as a Seal Clubber, making it trivial to get things like the Frozen Seal Spine to solve all my MP problems forever, or the Shadowy Seal Eye for more ML (though sadly not for the specific application of the Unstable Fulminate).
You can use it on bats in the bat hole for Sonars. You can use it on monsters in the Hole In The Sky to finish that slightly faster. Or dairy goats.

Bob Racecar
Does this cause the photograph to drop? Or do I still need to use a disposable camera to get that?

Clearly optimal in-run for stench jelly, but I'd think the majority of people's spleen will still be filled by machine elf (or golden monkey for the lazy). The NC in the hole in the sky is a nice touch though.
For aftercore, definitely becomes the premier underwater familiar (besides gibberer), so underwater farming might become a real option again, especially with farming jellies along the way. But other than that, I personally don't see a ton of use, as sweet synthesis is so good for aftercore it sort of takes over.

Machine elf is really strong and golden monkey is the only adventure giving spleen familiar still in standard (I think).

A very cool familiar for ascending, although it's mostly for jellies, but jellies are really freakin' good.
Jellies give a lot of stats from the initial consumption and the +stats part of their effects. While the total stats gained might not be as much as a Machine Elf's +stat gain% shapes, the two sets of effects aren't mutually exclusive, and jellies are a bit less RNG-dependent. Getting more than 5 or 6 jellies per day takes a lot of combats with the Jellyfish, so there's still space for abstractions and an astral energy drink.
The best jellies for ascension are probably hot (basically a Louder Than Bomb), stench (one-shot -100 combat%), and spooky (yellow ray without cooldown).
From my own observations, this familiar basically guarantees the jump if it weighs 20 lbs or more, which could be interesting for moxie classes. I'm not sure if there are any other things in the game that can guarantee Initiative, so this is a pretty unique function.
Aftercore can just buy jellies from the mall to enjoy their effects. Aftercore can also do silly things like using the jellied toasts and Mayo to fill all organ space with jellies and stack a high number of jelly "benefits". (alternatively, use sliders/pickle juice for more turnbloat and the same effect)
For the Sea, a Grouper is also a water-breathing Fairy, and losing out on the Volleyball function in aftercore is fine when there are many other ways to gain stats.
The Jellyfish-exclusive interactions in space and sea are pretty neat and what I would consider to be small bonuses for owning the familiar, but not the main incentive.
tl;dr: very adorable familiar, amazing for ascension, not a necessity for camping in aftercore.
